The Teaching Lifestyle Changes (TLC) department at Houston Northwest Medical Center has a diabetes self-management program available. It offers individual and/or group counseling and education. The program helps improve the self-care skills of people with diabetes and enhance the overall quality of their lives. It requires a doctor's order.
