Cardiopulmonary rehabilitation, also known as cardiac rehab, is a multiple-intervention program designed to assist individuals in maximizing quality of life. Due to the understanding that an individual's lifestyle can affect that progression of heart disease, the program utilizes a multidisciplinary approach in several areas of lifestyle that can include physical activity, nutritional habits, life stress management and smoking behavior. The program includes three phases and is six to twelve weeks in length, depending on the participant's progress.
Phase I (Inpatient Cardiac Rehab) occurs while the patient is hospitalized. Pertinent risk factors are discussed that may include appropriate physical activity, meal planning, stress management and smoking cessation (when appropriate). Education and support for family members is also offered.
Phase II (Outpatient Cardiac Rehab) begins shortly after discharge from the hospital with a physician’s order and individualized assessment and planning. This phase includes group education and monitored prescribed physical activity. During this phase participants begin an outpatient-monitored program that includes an exercise component, risk factor education, development of a home activity program and meal planning.
Phase III (Continuation of Outpatient Cardiac Rehab) is offered to those who need continued management through the Teaching Lifestyle Changes (TLC) program. The TLC program places strong emphasis on a lifelong exercise regime and positive lifestyle choices.
