Many of the patient care standards we focus on have been studied and developed by respected national health care organizations and institutions. As part of our commitment to improving quality of patient care at our hospital, we participate in several national programs and initiatives organized by these independent, third-party agencies to which we voluntarily report clinical outcomes data.
 
100,000 Lives Campaign


The 100,000 Lives Campaign was an initiative developed by the Institute for Healthcare Improvement (IHI) to engage U.S. hospitals in a commitment to implement changes to improve patient care and prevent avoidable deaths. The 3,100 hospitals nationwide that participated in this initiative saved an estimated 122,000 lives in 18 months. Houston Northwest Medical Center has implemented all six of the campaign’s quality improvement measures:

We have just begin participating in the IHI’s follow-up initiative, The 5 Million Lives Campaign , which challenges American hospitals to adopt six additional changes in care that save lives and reduce patient injuries
 
American Heart Association – Get with the Guidelines Program
 
Get With The GuidelinesSM is a hospital-based quality improvement program designed by the American Heart Association and the American Stroke Association to provide hospital staff with tools that follow proven evidence-based guidelines and procedures for cardiac patients in their care. It empowers health care provider teams to consistently treat heart and stroke patients according to the most up-to-date guidelines.
 
 
 
 
   
   
 ACTION Registry®–GWTG™ is a risk-adjusted, outcomes-based quality improvement program that focuses exclusively on high-risk STEMI/NSTEMI patients. It helps hospitals apply ACC/AHA clinical guideline recommendations in their facilities and provides invaluable tools to measure care and achieve quality improvement goals  
Hospitals awarded this designation have Primary PCI available 24/7 every day of the year. This means they have a call team made up of the cardiac cath lab staff and an interventional cardiologist who arrive to the facility within 30 minutes of STEMI activation. These facilities have formal agreements with the facilities that regularly refer STEMI patients for primary PCI. They will also have mapped their STEMI processes to maximize efficiencies which lead to quicker treatment and improved outcomes.  
 

PRIMARY STROKE CENTER

Houston Northwest Medical Center is recognized as a certified Primary Stroke Center for their exceptional efforts to achieve better outcomes for stroke care. Certification ensures those suffering stroke symptoms will be able to receive the care they need close to home, avoiding a long transport to another hospital when time is of the essence.

Houston Northwest Medical Center has been recognized as a 2013 Top Performer on Key Quality Measures® by The Joint Commission, the leading accreditor of health care organizations in the United States. Houston Northwest Medical Center was recognized as part of The Joint Commission’s 2014 annual report “America’s Hospitals: Improving Quality and Safety,” for attaining and sustaining excellence in accountability measure performance for Core Measures from CMS including AMI, HF, Surgical Care, Venous Thromboembolism and Stroke Care.
