To obtain copies of your records, please call our Medical Records Department at 281-440-2352 during regular business hours, Monday - Friday from 8 a.m. to 5 p.m.
Birth Certificates
Birth certificates may only be obtained through the Houston Department of Health and Human Services Bureau of Vital Statistics. The Bureau of Vital Statistics office is located at:
